Ideally, our mouth has to have 28 teeth; 14 in each jaw. Placing pterygoid implants in the upper jaw allows extremely good support to the back molars, without the need for sinus lift. Similarly basal implants in the lower jaw gives good support to back molars, without the need for bone grafting. 2nd molars are usually not replaced in many techniques where implants used are less, or because of surgeons limitations. Not replacing molars leads to more strain on the TMJ, as patient is forced to chew with front teeth.
